CHP+ Network members use the CHP+ State Managed Care Network Formulary administered by Colorado Access. Colorado Access' Pharmacy and Therapeutics Committee has selected medications on the Formulary for their quality and effectiveness. Please note that the medications on the Formulary are subjected to periodic review and amendment without notice.

When you choose to prescribe a generic medication, your patient will pay the lowest copayment available under their pharmacy benefit plan. When appropriate, please use formulary medications and choose the generic equivalent whenever possible.

The formulary is applicable to CHP+ members on the CHP+ Managed Care Network ONLY. If members are enrolled with one of the CHP+ participating HMOs, please refer to the individual HMOs formulary.
